Thyrotoxic periodic paralysis is a unique disorder that causes episodic proximal lower extremity muscle weakness in patient who already has hyperthyroid state and not treated for the underlying disease state. The affected population mostly includes Asian origin specifically male gender (vs most thyroid affects women gender). Precipitating factors include addition to are strenuous exertion, high-carbohydrate meal, drugs like diuretics, insulin, high dose of steroids, antiretrovirals, interferon therapy. Other genetic decreased activity Kir2.6 mutation increased Na+/K+ ATPase causing an intracellular shift potassium leads hypokalaemia. other challenging part physician differentiate it from Familial autosomal seen Caucasians Western countries because both disorders present similarly between them by presence with hypokalaemia paralysis. mortality associated respiratory hence acute intervention giving nonselective beta-blockers followed anti-thyroid medications.
